Clinical Registered Nurse (RN) — Min. 1 year exp.
Schedule: Day shift, Monday-Friday
Annual Salary: $76,960-$106,474

This RN opportunity will allow you to utilize your experience within a faith-based organization whom prides themselves on providing services to the local community. 

Primary Responsibilities:
  • As a member of the care team, provides basic medical case management to patients. This includes on an as needed basis: communicating with patients or families, helping execute a care plan, reconciling patient medications, providing interventions as appropriate, and helping with clinical follow up to ensure patient self-management or self-care goals are being addressed. 
  • Understands how role fits into a Patient Centered Medical Home model of care
  • Perform nursing care as appropriate to patients requiring wound care or other procedures falling within the RN’s scope.
  • Perform triage on patients that request same day or urgent appointments, providing nursing intervention when necessary, according to standing orders.
  • Participates in patient follow-up activities as directed by providers for normal and abnormal laboratory, DI or specialty consult results. Interacts with patient by phone, text, letter, or email/portal.
  • Receives and processes incoming calls per policy related to critical lab and DI results
  • Dispense medications according to approved standardized procedures, including PPD placement, nebulizer treatments and giving medication injections and vaccines. Administer medications in the clinic when appropriate.
  • Abide by HIPAA guidelines and policies, taking care to always protect patient information.
  • Prioritize daily workload to ensure that all responsibilities are carried out in a timely manner.

Minimum Requirements:
  • Diploma from an accredited nursing school as an RN. 
  • One year community or ambulatory health experience. Experience in case management.
  • Knowledgeable regarding nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to community health.
  • Relate to and work with the ill, disabled, elderly, emotionally upset, and at times, hostile people within the facility.
  • Demonstrate effective decision-making by assessing situations accurately, sharing fact-based information, communicating professional opinions clearly and inquiring actively.
  • Manage a high volume of patients in a short span of time, while dealing with unpredictable issues.
